| Additional HPI | The patient reports that he feels a ‘sharp burning sensation’ in his upper abdomen (points towards stomach). Very little relieves the pain and he is extremely concerned that the pain is getting worse. |
| Past medical hx | No insurance and has not been to a doctor in past 10 years |
| Surgical hx | Wisdom teeth removed age 23 |
| Family hx | Mother and Father both living. One older sister diagnosed with type II diabetes 1 year ago |
| Social hx | Works as teacher at elementary school and lives at home with wife and 2 children (healthy). Has been overweight for much of his life but has always been trying to eat healthy — no fast food, cooks well-balanced meals at home. |
