| Additional HPI | The boy is the youngest of 4 kids and the parents feel very strongly that he is far smaller than he should be at this age. He often is fussy about feeding and they feel that he is not progressing appropriately |

| Family hx | Mother and father are both healthy and siblings have had no serious issues |
| Social hx | Child was breast-fed for first 12 months and has been difficult to feed since then. Parents have tried numerous formulas that have all had limited success. |
